Problem
There was an issue with a PARIS Client that had two phone numbers of the same Type (in this case two OTHER PHONE numbers).

This caused the PARIS information to break i.e. the EMR Reg Form could not update the client record, and the user could not update the PARIS record. Script error: exact fetch returns more than requested number of rows.
A PARIS user cannot add two of the same types of phone number (i.e. home, work, other…); but in this case, the client had this on the record. The PARIS SA thinks this record may have been edited in the back end, which caused the problem.

Solution
Only one phone or address type should be active. All others should be end-dated or changed to a different type.
1. Ask user to identify which row should be end-dated in PARIS
2. If user does not have PARIS access, forward to VCH Community Data Quality (vchcommunitydataQuality@vch.ca) and copy Wong, Alice [VCH] <Alice.Wong@vch.ca>; Cain, Teresa [PH] <TCain@providencehealth.bc.ca> as well as the user.
