Excelleris requires an AUA form before it will send electronic results to the clinician.
- The Form should be completed once by the clinician. If the clinician is working at another VCH Profile EMR clinic, and is actively receiving results electronically, this form is not required. Excelleris just needs notification that a clinician is working at another clinic (see article: "Set up Clinicians to receive labs / transcriptions in EMR").
- It should be signed by the clinician, and then sent to Excelleris via email or via FAX as per the information at the bottom of the form.

To access this form:
- log in to Profile EMR
- Select icon on the Far right tool bar "Admin Forms"
- Select the item "Excelleris AUA Form"
- Complete the form with clinician information including MSP #(see screenshot below)
- Save as a PDF
- Clinician should sign the form in PDF format if possible, or print it and sign the form
- Send the form via email or fax to the instructions at the bottom of the form.
